All surgical training programs require time that is spent “on call.” This is the time, usually overnight or on the weekend, when a doctor is responsible for caring for hospitalized patients. It is also the time when new clinical problems emerge. These periods have exceptional educational value, but they are also a source of great stress. While on call, the doctor is usually one of the first to encounter significant changes in a patient’s condition and deviations in recovery patterns. Therefore, it is a unique time to improve clinical skills. However, under current training practices, a significant portion of the time spent on call is used early in the training process and the individual may not have encountered a variety of clinical situations.

On-call duty often requires working late hours and prioritizing numerous tasks. In addition, the on-call service usually includes “comprehensive coverage” or responsibility for patients with whom the on-call doctor may not be very familiar or informed. Often times, life-threatening changes in a patient’s condition can be hidden under seemingly harmless symptoms. Knowing and anticipating these issues can have a major impact on the patient’s outcome. It is therefore useful to have a plan for the quick but thorough and organized assessment and delivery of patient care. This book provides an overview of how to organize and implement care plans in response to many surgical readiness situations. It is written for interns and junior residents, but we hope the information is useful for individuals of all educational levels. Obviously, not all ready situations can be covered, but the emphasis has been placed on the more common and life-threatening problems.

Structure On Call Surgery 4th Edition

Most medical didactic training courses begin with a disease process in which the pathophysiology is explained first, followed by the clinical findings. Bedside training, however, begins with a physical complaint or a finding, and the underlying disease is only uncovered through attentive listening, observation and derivation. Then a broad differential diagnosis is made, which realizes that in order to make a diagnosis, you need to come up with a diagnosis.

Then differential diagnosis is reduced through medical history, physical exam, and the judicious use of laboratory and x-ray exams. In order to better follow the bedside mindset, the structure of On Call Surgery begins with the first call from a bedside nurse describing a patient problem. A list of likely diagnoses is made and a plan for evaluating and treating the patient is outlined. Particular attention is paid to those diseases that pose the greatest risk to a patient and the special needs of the pre-and post-operative patient.
